Comments on activities and operations observed:

- Preparations for connecting sections of pipeline located on the east riverbank;
- Application of coating on a section of pipe to be installed in the trench on the east riverbank;
- Backfilling the section of pipeline in the trench, on the west riverbank;
- Compacting for the area of trench backfilled with a bed of sand located inside the riparian protective strip west of the river;
- Installation of a pipeline identification tape and sand backfill, west side.

Observations

- A worker compacting sand in the trench west of the river was not wearing hearing protection, constituting non-compliance with the Construction Safety Manual for the project, which requires that workers exposed to loud noises wear hearing protection equipment (Action Plan, (Schedule 1, page 5 of 14), as required under section 20(1) of the OPR.

The NEB audit team attended the MACO meeting at the start of the day in the Champion offices at the construction site. The following topics were addressed: IPE requirements for tasks to be completed during the day, adjustments to be made to equipment and work methods owing to weather forecast (heavy rain) and a review of the activities completed the previous day. Several reminders were issued to the workers: first aid measures, accessibility of first aid responders and medical services, and environmental protection precautions.

Wednesday, November 11, 2015 (11:00 a.m. to 4:30 p.m.) and Thursday, November 12 (7:30 to 10:30 a.m.)
- Champion, Gaz Métro and MACO workers were wearing the required IPE appropriate to the tasks being performed.
- Workers were wearing eye and face protection for the work observed.
- Buried facilities “locates”; facilities located and identified.
- Gaz Métro representatives present at the site to ensure that work complied with requirements.
- Overhead power lines identified via signage.
- Construction site marked off to restrict access to the site.
- Signalers or workers at the site directing equipment operators.
- No workers present under loads being lifted.
- Excavations fenced and signage present.
- Trench boxes used at connection sites on the east and west sides of the river.
- Trench with wall shoring is kept sufficiently dry for the work in progress.
- No issues with respect to organization, cleanliness and maintenance (housekeeping) were detected.
- The fire extinguishers on site were certified and had the requisite monthly verification. The extinguishers were recorded on a HATCH equipment list and the annual certification and monthly verification labels were present on the extinguishers checked by the NEB Officer.

10:45 a.m.

Meeting with the Gaz Métro / Champion representative at the Thorne delivery station.
- Signage present; shows the station name, company name, operator name and emergency contact number (bilingual signage).
- Signage on the fence and inside the station shows location of emergency exits, warnings, fire protection, individual protective equipment and site safety.
- The pipes, accessories, valves, safety devices and other stationary equipment used to transport hazardous substances (gas and mercaptan), are clearly identified and the direction of flow is indicated by arrows on self-adhesive labels.
- An emergency exit is present and operational on the west side of the station. The emergency exit opens onto a steep slope covered with large stones and there is a stairway for accessing a safe site outside the station in the event staff had to evacuate in an emergency.
- A properly equipped emergency spill kit is located on site.
- Fire extinguishers at the station are full and have up-to-date monthly inspection labels.

Regulatory Requirements

The working-at-heights procedure (page 51) of the Construction Safety Manual for the project (page 6), states that the ladder must extend beyond the top level by three rungs and be firmly attached if it is used as a means of access. This is required under section 20(1) of the OPR, which states that “A company shall develop a construction safety manual and shall submit it to the Board.”

The NEB Officer considers this to be a corrected non-compliance (CNC) (1516-407 - CNC No.1), because the ladder was attached at the NEB Officer’s request.
